Job summary

All Metro Health Care, a Modivcare Personal Care Service, is seeking a Care Coordinator Associate in Lynbrook, NY. You will coordinate field staff schedules, communicate with patients and families, and document visits in the scheduling system to ensure timely care.

Responsibilities include backup support for Intake and maintaining clear communication across teams, with a focus on accuracy and patient satisfaction.

Qualifications

• High School Diploma or GED required.
• 2+ years of customer service experience required.
• 1+ years of home healthcare or related scheduling experience preferred.
• Electronic Visit Verification (EVV) system experience preferred.
• Bilingual in English & Spanish is a plus.

Responsibilities

• Assign field staff schedules and confirm assignments with employees and patients.
• Speak with patients or families regarding schedules, call-offs, cancellations, and appointments.
• Document client data and visits in the scheduling system.
• Be the backup to the Intake department for receiving and coordinating home care service requests.
• Understand that communication is vital within the office and with clients' care teams.
