Humana Care Coach position available as a remote role designed to guide members toward resources that support wellness and independence. You will coordinate with clinical, community, and multidisciplinary teams to optimize health outcomes while ensuring compliance.

The role requires an associate degree and at least two years in direct adult patient care; training lasts four weeks with hours 8:30–5:00 EST, then 10:00–6:30 EST, Monday–Friday.

Qualificações

• Associate degree in any course.
• Minimum 2 years of experience in direct adult patient care.
• Experience interacting with members of the healthcare team.
• Strong computer proficiency and ability to learn electronic medical records.
• Exceptional communication and interpersonal skills.

Responsabilidades

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess member needs and develop individualized care plans.
• Coordinate services with clinical, community, and multidisciplinary resources to support health outcomes.
• Identify and resolve barriers to care and member independence.
• Monitor progress and adjust care strategies as needed.
• Maintain adherence to compliance in a regulated program.
• Learn multiple computer systems while engaging members in a structured environment.
