Job summary

CVAUSA is seeking an RN Care Manager to lead a POD of Medical Assistants and Health Coaches, delivering proactive, value-based cardiovascular care. You will coordinate care plans, monitor quality metrics, and collaborate with cardiology and primary care teams to optimize patient outcomes.

The role emphasizes clinical oversight, care transitions, and risk mitigation in a remote work setting, with responsibilities spanning triage, assessments, and patient education to reduce readmissions.

Qualifications

• Active RN license (unrestricted).
• Active compact RN license required; Illinois license a plus.
• 3–5 years nursing experience, preferably in cardiology or care management.
• Experience in value-based care, managed care or risk-based contracts.
• Strong clinical assessment and triage skills.
• Ability to work effectively within a multidisciplinary team.

Responsibilities

• Provide clinical oversight to a POD of Medical Assistants and Health Coaches.
• Ensure POD aligns with clinical protocols and value-based care objectives.
• Act as clinical escalation point for complex cases and ensure timely interventions.
• Monitor POD performance and adherence to program requirements.
• Lead care planning huddles to prioritize enrollments and engagement.
• Coordinate with cardiology, primary care and post-acute providers.
• Deliver end-to-end care management including triage and transitional care.
