Job summary

Cardiac Care Alliance (CCA) is seeking an experienced RN Care Manager for a 100% remote, hourly contract role to support high-acuity cardiac patients. You will perform clinical triage, develop care plans, manage transitions, and collaborate with care teams to reduce hospitalizations and improve outcomes.

The ideal candidate has an active RN license (TX or compact), 2+ years in cardiac care or care management, and strong communication and documentation skills.

Qualifications

• Active RN license in Texas or a compact state.
• 2+ years of cardiac care or care management experience.
• Knowledge of chronic conditions, especially heart failure.
• Excellent communication and documentation skills.

Responsibilities

• Perform comprehensive clinical assessments via phone.
• Develop and update individualized care plans for cardiac patients.
• Conduct clinical triage and determine escalation needs.
• Lead TCM post-discharge, including medication reconciliation.
• Provide patient and caregiver education.
• Coordinate with providers and community resources.
• Serve as the initial point of contact for escalations and provide clinical oversight to Care Coordinators.
• Document all patient encounters per CMS requirements.
• Assess home safety and social determinants of health barriers; elevate resource needs.
• Maintain proactive communication with cardiologists and PCP offices.
• Review dashboards to address care gaps.
