Job summary

CardioOne is seeking a Remote Care Manager to provide critical virtual care support and remote patient management from home. You will develop care plans, reconcile medications, educate patients, and coordinate with clinicians to keep patients healthy between visits.

Ideal candidates have compact RN licensing and experience in remote care management, with strong communication and documentation skills to build meaningful patient relationships from a remote setting.

Qualifications

• Compact RN licensing required or willingness to obtain compact licensing.
• Cardiology experience preferred.
• Remote Care Management experience preferred.
• Strong communication and documentation skills.
• Patient assessment skills for remote care management.
• Ability to build meaningful relationships with patients who would benefit from additional healthcare support at home.

Responsibilities

• Provide remote care management support for all CardioOne's virtual care services.
• Remote patient support includes disease-specific care plan development & ongoing management, medication reconciliation, education & coaching, documentation and communication with clinicians.
• Respond to critical escalations of measurements and coordinate changes in care.
• Provide support to assigned LPN Care Coordinators and assist in onboarding new team members.
• Assist the Manager of Virtual Care Services with any clinical operations tasks as needed.
