Job summary

CardioOne is seeking a Virtual Care Manager to provide remote care management for our virtual care platform. You will work remotely, report to the Manager of Virtual Care Services, and help keep patients healthy between office visits through disease-specific care plans, coaching, and timely interventions.

The role emphasizes strong communication, documentation, and the ability to build relationships with patients at home, often coordinating with LPN Care Coordinators and clinicians.

Qualifications

• Compact RN licensing required or willing to obtain compact license.
• Cardiology experience preferred.
• Remote care management experience preferred.
• Strong communication and documentation skills.
• Patient assessment skills for remote care management.
• Ability to develop meaningful relationships with patients at home.

Responsibilities

• Provide remote care management support for CardioOne's virtual care services.
• Develop disease-specific care plans and ongoing management, with education and coaching, and documentation with clinicians.
• Respond to critical escalations of measurements and coordinate changes in care.
• Provide support to assigned LPN Care Coordinators and assist in onboarding new team members.
• Assist the Manager of Virtual Care Services with clinical operations tasks as needed.
