Job summary

Cardiac Care Alliance (CCA) seeks experienced RN Care Managers to support a high-acuity population of patients with complex cardiac conditions.

This is a full-time, remote, hourly contract position focused on delivering advanced clinical support through clinical triage, care plan development, and high-risk care management, including Transitional Care Management (TCM), Principal Care Management (PCM), Complex Care Management (CCM), Remote Patient Monitoring (RPM), and other applicable clinical

Qualifications

• Active RN license in Texas or a compact state.
• 2+ years of cardiac care or care management experience.
• Knowledge of chronic conditions, especially heart failure and comorbidities.
• Excellent communication and documentation skills.

Responsibilities

• Perform comprehensive clinical assessments via phone.
• Develop and update individualized care plans for cardiac patients.
• Conduct clinical triage and determine escalation needs.
• Lead TCM post-discharge activities including medication reconciliation.
• Provide patient and caregiver education.
• Coordinate with providers and community resources.
• Serve as the initial point of contact for escalations and provide oversight to Care Coordinators.
• Document all patient encounters per CMS requirements.
• Assess home safety and social determinants of health barriers; escalate resource needs.
• Maintain proactive communication with Cardiologists and PCP offices.
• Review population health dashboards to address care gaps.
