Job summary

Cardiovascular Services of America (CVAUSA) is seeking a remote CCM Health Coach to support patients with chronic conditions, especially heart failure, by building purposeful coaching relationships and aligning care plans with the cardiology team.

You will use EMR tools to monitor adherence, communicate findings to providers, and help patients navigate enrollment while staying compliant with regulations and policies.

Qualifications

• High School diploma or GED required.
• 1 year of experience in remote chronic care management.
• Experience with cardiology patients, especially heart failure.
• Excellent communication and multi-tasking skills.

Responsibilities

• Develops a patient coaching relationship that supports the care plan and quality of life.
• Provides empathic support for patients and families dealing with chronic conditions.
• Communicates abnormal findings to the cardiology care team.
• Monitors and encourages adherence to care plans.
• Clearly communicates findings to the care team.
• Assists new patients with enrollment processes.
• Acts as liaison between patients, the practice and providers.
• Identifies changes to providers to reduce hospitalizations.
• Works with the CCM platform and EMR systems.
• Performs basic monitor troubleshooting with patients.
