Job summary

Cardiovascular Services of America (CVAUSA) is seeking a remote CCM Health Coach to support patients with chronic conditions. You will build coaching relationships, monitor adherence, and report findings to the cardiology care team while assisting new patient enrollment.

The role requires RN, MA, or CNA credentials along with EMR experience (Athena, NexGen, GEMMS, e-clinical works) and strong communication skills. This is a full-time, remote position with US-based coverage.

Qualifications

• RN, MA or CNA credentials required or preferred.
• Experience with remote chronic care management.
• Experience with cardiology patients, especially heart failure.
• Strong communication and organizational skills.

Responsibilities

• Develops a patient coaching relationship to support the care plan and quality of life.
• Communicates abnormal findings to the cardiology care team.
• Monitors and encourages adherence to care plans.
• Acts as liaison for patients with the practice and providers.
• Escalates changes to providers to reduce hospitalizations or urgent care visits.
