Job summary

Cardiac Care Alliance is seeking experienced Registered Nurse (RN) Care Managers to support high-acuity cardiac patients. This is a full-time, remote, hourly contract role focused on clinical triage, care plan development, and high-risk care management including RPM and other programs.

Qualifications

• Active RN license in Texas or a compact state.
• 2+ years of cardiac care or care management experience.
• Knowledge of chronic conditions, especially heart failure.
• Excellent communication and documentation skills.

Responsibilities

• Perform comprehensive clinical assessments via phone.
• Develop and update individualized care plans for cardiac patients.
• Conduct clinical triage and determine escalation needs.
• Provide patient and caregiver education.
• Coordinate with providers and community resources.
• Document encounters per CMS requirements.
• Support care coordinators and clinical escalation as needed.
