Job summary

seamlessassist is seeking a remote Clinical Pharmacist with a cardiology focus to support value-based care programs. This full-time contract role emphasizes GDMT optimization, medication titration, and patient education, collaborating with RN Care Managers and cardiology providers to improve outcomes for high-risk patients.

You will conduct telephonic medication reviews, review profiles, and document recommendations in the EMR.

Qualifications

• PharmD degree and active Texas license in good standing.
• Multi-state licensure required for remote practice.
• 3–5 years of clinical cardiology experience and GDMT expertise.
• Strong telephonic patient communication and documentation.
• Comfort working in a nurse-led care management model and with cardiology providers.
• Ability to work CT business hours across US time zones.

Responsibilities

• Review cardiology patient profiles for GDMT optimization and titration.
• Provide telephonic patient education on cardiac meds and adherence.
• Collaborate with RN Care Managers and Cardiologists to align medication plans.
• Document recommendations promptly in the EMR / care management platform.
• Support quality measures and reduce adverse medication events.
