Qualifications

• PharmD degree required.
• Active Texas pharmacist license in good standing.
• Multi-state licensure required (remote role, not Texas residency restricted).
• Minimum 3–5 years of clinical experience with a strong cardiology focus.
• Demonstrated working knowledge of GDMT and cardiac medication titration.
• Strong telephonic patient communication skills.

Responsibilities

• Review patient medication profiles for cardiology populations (heart failure, CAD, AF, hypertension).
• Evaluate GDMT appropriateness based on ACC/AHA guidelines and payer programs.
• Identify gaps, contraindications, drug interactions, and adherence barriers.
• Make evidence-based recommendations for initiation, optimization, and titration with providers and nursing staff.
• Conduct telephonic medication reviews and patient education.
