Job summary

Seamless Assist is seeking a full-time, remote Clinical Pharmacist with a cardiology focus to support value-based care programs. This role emphasizes GDMT optimization, medication titration, adherence, and collaboration with nurses and cardiology providers to improve outcomes for high-risk cardiac patients.

This is not a dispensing role. You will work telephonically within a nurse-led care model, joining RN Care Managers and Cardiology APPs, ensuring documentation in the EMR and care management

Qualifications

• PharmD with active license (Texas) and multi-state eligibility.
• 3–5 years of clinical cardiology experience required.
• Strong knowledge of GDMT and cardiac medication titration.
• Excellent written and verbal English; telephonic patient communication.

Responsibilities

• Review cardiology patients' meds and optimize GDMT per guidelines.
• Conduct telephonic patient reviews and educate on cardiac meds.
• Collaborate with RN care managers, nurses, and cardiology providers.
• Document recommendations promptly in the EMR/care management platform.
