Job summary

Seamless Assist is seeking a remote Clinical Pharmacist with a cardiology focus to optimize GDMT, support titration, and improve adherence for high-risk cardiac patients. You will work within a nurse-led care model, partnering with nurses and providers across time zones to deliver patient-centered care.

The role is full-time, contract, 40 hours weekly, requiring multi-state licensure (including Texas) and a strong cardiology background.

Qualifications

• PharmD degree required with active Texas license and multi-state licensure.
• 3–5 years of clinical cardiology experience focusing on GDMT.
• Strong telephonic patient communication and collaboration with cardiology teams.

Responsibilities

• Review patient meds for GDMT optimization in cardiology populations.
• Provide telephonic education and support to patients on GDMT and titration.
• Collaborate with RN Care Managers, Nurse Navigators and cardiologists to align therapy.
• Document recommendations promptly in the EMR and care management platform.
