Job summary

Seamless Assist seeks a Clinical Pharmacist with strong cardiology expertise to optimize GDMT and titration for high-risk patients. This fully remote, contract role collaborates with nurses and cardiology providers to improve outcomes, emphasizing telephonic patient engagement and adherence.

You will work within a nurse-led care model, reviewing therapies, addressing gaps, and documenting recommendations in the EMR to support value-based care programs.

Qualifications

• PharmD required and active Texas license; multi-state licensure needed for remote role.
• Minimum 3–5 years of clinical cardiology experience.
• Strong knowledge of GDMT and cardiac medication titration.
• Comfort working in a nurse-led, interdisciplinary care model.
• Excellent written and verbal English communications; telephonic interactions.
• Remote, fully remote role with US-wide coverage.

Responsibilities

• Review patient cardiology medication profiles and GDMT optimization.
• Collaborate with nurses and cardiology providers on titration and adherence.
• Conduct telephonic medication reviews and patient education.
• Document recommendations promptly in the EMR/care management platform.
