Job summary

Seamless Assist is seeking a full-time, contract Clinical Pharmacist with strong cardiology experience to support value-based care programs. This remote role focuses on GDMT optimization, medication titration, and adherence, collaborating with nurses and cardiology providers to improve outcomes for high-risk cardiac patients.

This is a non-dispensing clinical partner embedded within a nurse-led care team. The pharmacist will review therapies, communicate recommendations, and document in the EMR,

Qualifications

• PharmD degree and active Texas pharmacist license.
• 3–5 years of clinical cardiology experience.
• Multi-state licensure required for a remote role.
• Strong GDMT knowledge and medication titration experience.
• Comfort working in a nurse-led, interdisciplinary care model.
• Excellent telephonic patient communication skills.

Responsibilities

• Review patient med profiles for cardiology populations and assess GDMT optimization.
• Make evidence-based recommendations for initiation, optimization, and titration.
• Engage with patients via telephonic counseling and education on GDMT.
• Collaborate with RN Care Managers, Nurse Navigators, and cardiologists to align plans.
• Document recommendations and patient interactions in the EMR promptly.
• Support quality measures in value-based care and reduce adverse events.
