Job summary

seamlessassist is seeking a Clinical Pharmacist with strong cardiology experience to support value-based care programs in a fully remote capacity across the United States. The role focuses on GDMT optimization, medication titration, adherence improvement, and close collaboration with nurses and cardiology providers to improve outcomes in high-risk patients.

This is a contract, full-time position with 40 hours per week.

Qualifications

• PharmD with active Texas license and multi-state licensure.
• Minimum 3–5 years of clinical cardiology experience.
• Strong GDMT knowledge and titration experience.
• Comfort working telephonically in a nurse-led care model.
• Excellent written and verbal English communication.

Responsibilities

• Review patient profiles for GDMT optimization in cardiology populations.
• Perform telephonic medication reviews and patient education.
• Educate patients on purpose, benefits, and side effects of cardiac meds.
• Collaborate with RN Care Managers, Nurses, and Cardiologists; document in EMR.
• Support quality measures and adherence initiatives in value-based care.
