Job summary

Seamless Assist seeks a full-time, remote Clinical Pharmacist with cardiology focus to optimize GDMT, titrate therapies, and improve adherence in a value-based care setting. You will work with nurses and cardiology providers to support high-risk patients and overall outcomes.

The role emphasizes non-dispensing clinical practice, telephonic patient engagement, and collaboration within a nurse-led care model across the US.

Qualifications

PharmD degree and active pharmacy license in the United States.
Multi-state licensure required; remote role not restricted to one state.
Minimum 3–5 years of clinical cardiology experience.
GDMT knowledge and cardiac titration experience.
Comfort working in a nurse-led, interdisciplinary care model.
Strong telephonic patient communication skills.
Ability to interact professionally with cardiology providers.
Must hold Texas license active.

Responsibilities

Review patient medication profiles for cardiology populations and optimize GDMT and titration.
Conduct telephonic medication reviews and patient education.
Collaborate with RN Care Managers, Nurse Navigators, and cardiologists to align medication plans.
Document recommendations and patient interactions in the care management platform / EMR; support quality measures.
