Job summary

SeamlessAssist is seeking a full-time, remote Clinical Pharmacist with strong cardiology experience to support value-based care programs. Focus on optimizing GDMT, medication titration, and adherence; collaborate with nurses and cardiology providers in a nurse-led care model.

This role is non-dispensing; involves telephonic patient reviews, education, and documentation in the care management platform/EMR. Must hold multi-state licensure including Texas and be able to work CT business hours.

Qualifications

• PharmD degree and active Texas license.
• Multi-state licensure required; remote role.
• 3–5 years of clinical cardiology experience.
• Strong GDMT knowledge and cardiac medication titration.
• Excellent telephonic patient communication.
• Ability to interact professionally with cardiology providers.
• Time zone compatibility with CT business hours.
• Non-dispensing clinical role in a nurse-led care model.

Responsibilities

• Review cardiology patient medication profiles and optimize GDMT.
• Conduct telephonic medication reviews and patient education.
• Collaborate with RN Care Managers and cardiology providers.
• Document recommendations in EMR / care management platform.
• Support quality measures in value-based care programs.
