Job summary

Seamlessassist is seeking a full-time, contract Clinical Pharmacist with strong cardiology experience to support value-based care programs. This remote role focuses on optimizing GDMT, supporting medication titration, improving adherence, and collaborating with nurses and cardiology providers to improve outcomes for high-risk cardiac patients.

The position is fully remote across the US, requires multi-state licensure (Texas license active), and emphasizes team-based care in a nurse-led model.

Qualifications

• PharmD with active Texas license and multi-state eligibility.
• 3–5 years of clinical cardiology experience.
• Strong knowledge of GDMT and cardiac medication titration.
• Ability to work within a nurse-led, interdisciplinary care model.
• Excellent written and verbal communication in English.

Responsibilities

• Review patient cardiology therapies and optimize GDMT.
• Titrate medications and support adherence.
• Conduct telephonic patient reviews and education.
• Collaborate with nurses and cardiologists; document in EMR.
