Job summary

Seamless Assist is seeking a full-time, remote Clinical Pharmacist with cardiology expertise to optimize GDMT, titrate medications, and enhance adherence within a value-based care framework.

This role is non-dispensing and embeds the pharmacist within the care team, collaborating with nurses and cardiology providers to improve outcomes for high-risk cardiac patients, while maintaining multi-state licensure requirements.

Qualifications

• PharmD degree and active Texas pharmacist license required.
• Multi-state licensure (remote role) required.
• Minimum 3–5 years of clinical cardiology experience.
• Strong GDMT knowledge and cardiac medication titration.
• Excellent telephonic patient communication and collaboration with care team.

Responsibilities

• Review patient medication profiles for cardiology populations and GDMT optimization.
• Conduct telephonic medication reviews and patient education.
• Collaborate with RN Care Managers, Nurse Navigators, and cardiology providers.
• Document recommendations promptly in the care management platform / EMR.
• Support quality measures in value-based care programs.
• Operate in a non-dispensing clinical role, focusing on care coordination.
