Job summary

Seamless Assist is seeking a Clinical Pharmacist with strong cardiology experience to support value-based care programs. This is a fully remote, full-time contract role focusing on GDMT optimization, titration, adherence, and collaboration with nurses and cardiology providers to improve outcomes for high-risk cardiac patients.

This is a non-dispensing clinical role embedded within the care team. The role requires a PharmD, active Texas license with multi-state licensure, 3–5 years of cardiology

Qualifications

• 3–5 years of clinical cardiology experience.
• Strong GDMT knowledge and medication titration expertise.
• Ability to work telephonically within a nurse-led care model.
• Excellent written and verbal English communication.

Responsibilities

• Review patient medication profiles for cardiology populations and GDMT optimization.
• Educate patients on purpose, benefits, side effects, and expectations of cardiac medications.
• Collaborate with nurses, cardiologists, and APPs to align medication plans with care goals.
• Document recommendations and patient interactions in the care management platform / EMR.
• Support quality measures and reduce medication-related adverse events.
