Job summary

Seamless Assist is seeking a remote Clinical Pharmacist with strong cardiology GDMT experience to optimize guideline-directed medical therapy and titration for high-risk patients. You will collaborate closely with RN Care Managers, Nurse Navigators, and cardiology providers to close therapy gaps and improve adherence.

This W-2 role emphasizes clinical partnership in a nurse-led care model, with non-dispensing duties and emphasis on remote, telephonic patient interactions.

Qualifications

• PharmD degree required.
• Active, unrestricted Texas pharmacist license at time of hire.
• 3–5+ years of clinical pharmacy with cardiology focus.
• Hands-on GDMT knowledge and titration experience.
• Familiarity with ACC/AHA guidelines.
• Comfort in a nurse-led care model.
• Strong telephonic communication skills.
• Excellent written English.

Responsibilities

• Review cardiology med profiles and GDMT regimens for optimization.
• Lead GDMT assessments against ACC/AHA guidelines and payer programs.
• Identify gaps, contraindications, and adherence barriers and address them.
• Collaborate with providers and nursing staff to titrate therapies.
• Document recommendations promptly in the care management platform/EMR.
• Educate patients via telephonic reviews and education.
