Cadence, a fully remote clinical AI company, seeks a Nurse Practitioner to lead clinical decision-making for remote heart failure, diabetes, and hypertension care. You will coordinate with physicians, RNs, and care teams to titrate GDMT and optimize treatment plans while ensuring timely, patient-centered care.

This role emphasizes evidence-based guidelines, strong clinical autonomy, and collaboration within a fast-paced remote environment.

Competenze

• Active NP licensure and RN licensure in good standing.
• 5+ years treating chronic diseases (CHF, HTN, T2D) in high-performing centers.
• ANCC or AANP certification.
• Willingness to expand state licensure as Cadence expands markets.

Mansioni

• Manage a panel of remote care patients with GDMT for heart failure, HTN, and T2D.
• Follow clinical protocols while applying individual clinical judgment.
• Titrate GDMT in collaboration with physicians and care teams.
• Respond to remote monitoring alerts and abnormal vitals.
• Provide synchronous and asynchronous care including virtual visits and secure messaging.
