Job summary

Cadence, a clinical AI company delivering remote chronic care, is hiring a Nurse Practitioner to be the primary clinical contact for patients navigating their remote care journey. You will lead clinical decisions with a focus on guideline-directed therapy for heart failure and comorbid diabetes and hypertension.

This fully remote role requires high clinical autonomy, collaboration with RNs and care teams, proactive patient management, timely decisions, and accurate documentation in a fast-paced

Qualifications

• Active NP licensure with 2+ states (TX, MI, NC, CA, PA, NJ, CT, AL)
• Active Compact Multi-State RN license
• 5+ years treating chronic diseases (CHF, HTN, T2D)
• 3+ years cardiology with GDMT focus
• ANCC or AANP certification
• Willingness to expand licensure as Cadence grows
• Strong home-office setup with stable Wi‑Fi

Responsibilities

• Manage a panel of patients receiving remote care and make timely clinical decisions based on guidelines for heart failure, hypertension, and T2D
• Deliver evidence-based care following clinical protocols
• Initiate and titrate GDMT with cardiology and care teams
• Respond to remote patient monitoring alerts and abnormal vitals
• Provide synchronous and asynchronous patient care with secure messaging and virtual visits
• Maintain productivity and thorough documentation
• Adapt to evolving workflows and technology
• Collaborate across clinical and operational teams to improve patient care and scalability
