Job summary

Cadence is a clinical AI company automating chronic disease treatment. We are hiring a Nurse Practitioner to be the primary clinical contact for patients navigating their remote care journey, with a focus on GDMT for heart failure and guideline-based care for T2D and hypertension.

The role is fully remote, with expectations for productivity, rapid collaboration with care teams, and ongoing adaptation to evolving workflows and technology. Commensurate compensation is provided.

Qualifications

• Active Compact Multi-State RN license.
• Active NP licensure in CA, WA, or AZ preferred.
• Master’s Degree as a Nurse Practitioner with willingness to expand licensure.
• 5+ years treating chronic diseases (CHF, Hypertension, T2D).
• 3+ years in cardiology focusing on GDMT.
• ANCC or AANP certification is required.

Responsibilities

• Manage a panel of patients receiving remote care with timely decisions per guidelines.
• Deliver evidence-based care following established protocols and workflows.
• Initiate and titrate GDMT with physicians and care teams.
• Respond quickly to escalations from remote monitoring (vitals, symptoms, labs).
• Provide synchronous and asynchronous care, review patient data, and conduct virtual visits.

3+ years cardiology GDMT

Education

Master’s Degree as a Nurse Practitioner

Active ANCC or AANP certification
