Job summary

Cadence, a clinical AI company, seeks a Nurse Practitioner to serve as the primary clinical contact for patients on remote care. You will lead decisions on GDMT for heart failure and guidelines for T2D and hypertension, with asynchronous titration and close collaboration with physicians and care teams.

The role is fully remote, Pacific Time-aligned, with strong emphasis on productivity, quality, and collaboration across RNs and care teams. Requires licensure and experience in cardiology.

Qualifications

• 5+ years treating patients with chronic diseases (CHF, Hypertension, T2D).
• Cardiology focus on heart failure management and GDMT.

Responsibilities

• Manage a panel of patients receiving remote care and make timely clinical decisions.
• Deliver evidence-based care following established protocols and workflows.
• Initiate and titrate guideline-directed medical therapy with physicians and care teams.
• Respond to clinical escalations from remote patient monitoring.
