The NP must have a Master’s degree in Nursing, an active Compact Multi-State RN license, NP licensure in two states, ANCC/AANP certification, and CHF-focused cardiology experience.

Qualifications

• Five+ years of experience treating chronic disease.
• Three+ years in cardiology focusing on heart failure management and guideline-directed medical therapy.

Responsibilities

• Manage a remote patient panel with evidence-based care for heart failure, hypertension, and type 2 diabetes.
• Review remote-monitoring alerts for vital signs, symptoms, and labs.
• Conduct synchronous and asynchronous care via secure messaging and virtual visits.
• Meet productivity and documentation expectations for the patient panel.
