Job summary

Good Shepherd Services, Ltd. is seeking a dedicated Bilingual (Spanish-speaking) Care Coordinator for our Health Home program in Brooklyn, NY. The on-site role may include hybrid scheduling based on program needs.

You will guide youth and families through healthcare access, coordinate services, and monitor outcomes to ensure high-quality care. The position reports to the Care Manager Supervisor, Health Services and requires strong advocacy and interdisciplinary collaboration.

Qualifications

• Bachelor's degree required with two years' experience
• Bilingual Spanish/English candidates encouraged to apply
• Experience as a Case Planner, Care Coordinator, Case Manager, or Case Worker in social services
• Experience with Child Welfare; ACS; and Foster Care helpful

Responsibilities

• Secure all required Care Management enrollment consents from the individual or guardian.
• Conduct initial and ongoing comprehensive needs assessments (CANS) to determine level of care management.
• Oversee IPC development and management, ensuring services match enrollee acuity and treatment plans.
• Meet client contact requirements, including face-to-face visits for high/medium/low acuity.
• Complete Care Management documentation in MAPP and EHRs accurately and timely.
• Advocate for clients within the agency and with external providers.
• Promote wellness by linking enrollees to resources and services.
• Educate children and caregivers on chronic conditions, immunizations, and preventive care.
• Assist clients in accessing Medicaid, cash assistance, SNAP, housing, and other supports.
• Communicate effectively with families, respecting language, literacy, and culture.
• Facilitate care planning meetings and participate in interdisciplinary teams.
• Identify community resources and manage referrals and follow-up services.
• Participate in hospital discharge planning for post-discharge services.
• Attend ongoing staff development to maintain Care Management skills.
• Ensure EPSDT-related evaluations and follow-ups for children.
