Job summary

Healthfirst is seeking a Relationship Coordinator to educate and assist prospective members about Long-Term Care enrollment across MAP, MLTC SHP, and HARP programs. The role emphasizes accurate product knowledge, timely follow-up, and meeting enrollment quotas in a paperless call center environment.

You will use Salesforce, SharePoint, and MS Office 365 to manage referrals, appointments, and documentation while interacting with diverse communities. Remote work capabilities are supported.

Qualifications

• High school diploma or GED.
• Fluency in reading and speaking English and Spanish.
• Member / Customer Service experience.

Responsibilities

• Handle inbound and outbound calls to address customer needs while meeting SLAs.
• Create/follow up LTC leads and opportunities in Salesforce (up to 90 days).
• Search Medicaid eligibility in ePACEs and CMS Marx for Medicare eligibility and elections.
• Achieve daily, weekly, and monthly growth goals.
• Schedule appointments for SHP Intake Nurses and HF staff in Salesforce.
