Job summary

Cadence Health is hiring a Bilingual/Spanish Speaking Clinical Navigator (Health Coach) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This remote, United States–based role focuses on proactive outreach, coaching, and care coordination.

You will use AI-assisted tools and collaborate with clinicians, documenting interactions in the Cadence platform, while ensuring patients feel supported and engaged throughout their care journey.

Qualifications

• NBHWC certification required.
• At least 1 year of health coaching experience.
• Experience delivering education and support to chronic conditions.
• Remote work readiness with reliable Wi‑Fi.
• Spanish fluency required.
• Ability to discuss AI-supported care with providers and patients.

Responsibilities

• Conduct outbound patient outreach and one-on-one coaching for chronic conditions.
• Guide patients through personalized care plans and behavior change goals.
• Assess health status and readiness to change.
• Educate on disease management, adherence, and prevention strategies.
• Monitor progress and update care plans using Cadence tools.
• Document interactions and connect patients with resources.
• Collaborate with clinicians and support teams in real time.
