Job summary

CVS Health is seeking a licensed Behavioral Health Case Manager (Bilingual) to deliver telephonic care management for Medicare members from a home office, Monday through Friday business hours.

You will conduct assessments, develop care plans, and coordinate with providers and community resources to improve member well‑being. A Master’s in Social Work or Counseling and a valid state license are required; Spanish/English fluency is essential for patient engagement and documentation.

Qualifications

• Active behavioral health license in your state (LCSW, LPC, LMFT, LPCC, LISW, LSW or equivalent)
• Master's Degree in Social Work or Counseling
• 3+ years of direct clinical behavioral health experience
• Experience with crisis intervention and motivational interviewing
• Bilingual in Spanish and English (read, write, speak fluently)
• Strong assessment and care planning skills
• Ability to work independently in a telephonic, computer-based environment
• High-speed internet (min 25 Mbps down / 3 Mbps up)

Responsibilities

• Provide telephonic case management for Medicare members with behavioral health needs
• Complete comprehensive assessments and develop individualized care plans
• Utilize motivational interviewing techniques to engage members and support positive health outcomes
• Coordinate care with providers, health plan partners, and community resources
• Assist members in accessing behavioral health, social, and community support services
• Provide behavioral health consultation and collaboration across internal teams
• Participate in clinical treatment rounds and interdisciplinary care discussions
• Contribute ideas to support program development and continuous improvement
• Maintain accurate documentation within clinical systems
