CVS Health is hiring a Bilingual Behavioral Health Case Manager (Licensed) to manage telephonically for Medicare members from home. The role emphasizes comprehensive care management, behavioral health advocacy, and coordination with providers and community resources.

Requires an active behavioral health license (LCSW/LPC/LMFT/LPCC/LISW/LSW or equivalent), a Master’s in Social Work or Counseling, and 3+ years of experience. Bilingual Spanish/English is essential.

Qualifications

• Licensed behavioral health professional with state-licensure.
• Master's Degree in Social Work or Counseling.
• 3+ years of direct clinical behavioral health experience.
• Spanish-English bilingual required.
• Strong assessment, care planning and engagement skills.
• Ability to work independently in a telephonic environment.
• High-speed internet required (25 Mbps down / 3 Mbps up).

Responsibilities

• Provide telephonic case management for Medicare members with behavioral health needs.
• Complete assessments and develop individualized care plans.
• Utilize Motivational Interviewing to engage members and support outcomes.
• Coordinate care with providers, health plan partners, and community resources.
• Assist members in accessing behavioral health, social and community services.
• Provide behavioral health consultation across internal teams.
• Participate in clinical treatment rounds and interdisciplinary discussions.
• Contribute ideas to program development and continuous improvement.
• Maintain accurate documentation within clinical systems.
