Job summary

Integral Health is hiring a Licensed Therapist / Behavioral Care Manager (Remote) to deliver therapy and care coordination within primary care teams.

You will conduct telehealth sessions, implement brief interventions, and monitor progress using standardized tools. The role emphasizes short-term, measurement-based care with strong documentation and collaboration with physicians and consultants.

Qualifications

• Master’s degree in a behavioral health field is required.
• Active NY LMSW/LCSW/LMHC/LMFT license required and must remain in good standing.
• At least two years of direct mental health therapy experience requested.

Responsibilities

• Provide direct, patient-facing behavioral health therapy via telehealth.
• Use brief, evidence-based interventions (CBT, motivational interviewing, problem-solving therapy, behavioral activation).
• Screen and assess for mental health conditions, substance use, safety risks, and SDOH.
• Utilize PHQ-9, GAD-7, C-SSRS, and SDOH screenings to guide treatment and track progress.
• Develop patient-centered care plans with SMART goals.
• Coordinate with PCPs and psychiatric consultants and document in EMR.
• Engage in supervision and contribute to clinical workflows.
