Job summary

Integral Health is seeking a Behavioral Care Manager to provide direct therapy to adolescents and adults via telehealth and to coordinate care within the Collaborative Care Model. The role blends short-term therapy with care management and requires a NY-licensed clinician.

You will work part-time remotely, with evenings and weekends expected for adolescent therapy. The position offers professional development and strong clinical supervision in a supportive environment.

Qualifications

• Master’s degree and active NY license in behavioral health.
• Minimum two years of full-time-equivalent direct mental-health treatment experience after licensing.
• Ability to commit to at least 15 hours per week.
• Experience delivering therapy to adolescents and adults.
• Comfort with telehealth and remote coordination.

Responsibilities

• Provide direct, patient-facing behavioral health therapy to adolescents and adults through telehealth.
• Deliver short-term, evidence-based interventions using CBT, motivational interviewing, problem-solving therapy, and behavioral activation.
• Conduct clinical assessments and develop focused, measurable treatment goals.
• Assess and respond to safety concerns, including C-SSRS screenings and escalating higher-acuity cases.
• Coordinate transitions to longer-term or specialty treatment when needed.
• Coordinate behavioral healthcare with PCPs and psychiatric consultants.
• Screen for and monitor depression, anxiety, substance use using PHQ-9, GAD-7, C-SSRS.
• Document therapy sessions, progress, and treatment recommendations in the EHR.
• Educate patients and families about diagnoses, symptoms, and treatment options.
