Job summary

Integral Health is hiring a Behavioral Care Manager (BCM) as a 1099, part-time contractor, working remotely with primary care teams to deliver collaborative care.

You will use brief, evidence-based interventions, monitor progress, and coordinate with physicians and psychiatric consultants. Requires NY license (LMSW/LCSW/LMHC/LMFT), adolescent behavioral health experience, and at least 2 years of licensed practice.

Qualifications

• Master's degree required and NY license (LMSW/LCSW/LMHC/LMFT) active.
• Minimum 2 years of full-time licensed mental health practice.
• Able to commit to at least 15 hours per week.
• Experience with Collabrative Care Model preferred.

Responsibilities

• Coordinate behavioral health care with primary care teams.
• Screen for depression, anxiety, substance use using PHQ-9, GAD-7, CSSRS.
• Monitor progress toward symptom reduction (goal: 50% or 10-point drop).
• Support PCP-led medication management and track adherence/side effects.
• Use patient registry to follow up and adjust care plans.
• Document progress and recommendations in the EHR.
• Join regular caseload consultations with a psychiatric consultant.
• Assist in relapse prevention plans when needed.
