Job summary

Integral Health seeks a Behavioral Care Manager (BCM) for a remote, part-time 1099 contract in New York. Bring LMSW/LCSW/LMHC/LMFT or equivalent licensure, 2+ years of licensed mental health experience, and ability to provide direct structured therapy to adolescents and adults.

Role emphasizes collaboration with primary care and psychiatric consultants within the Collaborative Care Model, with measurable outcomes and flexibility in scheduling.

Qualifications

• Master’s degree in a behavioral health field.
• Active New York license as LMSW/LCSW/LMHC/LMFT or equivalent eligible license.
• At least 2 years of full-time experience as a licensed mental health professional.
• Experience providing direct behavioral health treatment or individual therapy.
• Ability to commit to a minimum of 15 hours per week.
• Remote work capability with strong documentation skills.
• Experience using EHR/EMR systems and other clinical tools.

Responsibilities

• Provide individual behavioral health therapy and brief evidence-based interventions.
• Maintain an assigned patient caseload.
• Conduct behavioral health screenings (PHQ-9, GAD-7, C-SSRS).
• Develop and update individualized treatment plans with measurable outcomes.
• Collaborate with primary care providers and psychiatric consultants.
• Document patient progress and care coordination in the EHR.
• Participate in regular psychiatric caseload consultations and interdisciplinary discussions.
• Educate patients and families about conditions, options, and resources.
• Complete relapse-prevention planning for remission or treatment goals.
