Job summary

Triplemoon is hiring a bilingual Behavioral Health Care Manager (BHCM) to provide direct, patient-facing care to children, teens, and families within a Collaborative Care Model (CoCM). This fully remote role involves about 75% direct patient contact and delivering brief, evidence-based care designed to drive meaningful change within 6–12 months.

Qualifications

• Active, unrestricted licensure as a licensed clinical social worker (LCSW), licensed professional counselor (LPC), and/or licensed therapist (LMFT).
• Bilingual Spanish is a requirement for this role.
• 3+ years of experience OR a minimum of 3,000 supervised hours.
• Experience working with children, adolescents, and families.
• Experience in telehealth/e-Health platforms and care coordination.

Responsibilities

• Conduct intake assessments and ongoing follow-up sessions via telehealth.
• Deliver brief, evidence-based interventions (CBT, MI, BA) and parent coaching.
• Provide in-session education and actionable recommendations to families.
• Track patient progress using validated screening tools.
• Coordinate with schools, SDoH resources, and other services; collaborate with pediatricians and psychiatric consultants.
• Participate in weekly registry review meetings and document care in Healthie.
• Maintain scheduling, follow-ups, and patient engagement with standardized protocols.
