Job summary

Carbon Health is seeking a licensed Mental Health Specialist to join our Collaborative Care Model (CoCM) team in a fully remote role. You will coordinate care for an assigned caseload, conduct brief behavioral interventions, and monitor outcomes in collaboration with primary care and psychiatric consultants.

This position requires active California licensure and eligibility for Washington State licensing, with a schedule of Monday–Friday, 8:00 a.m.–5:00 p.m. and 40 hours per week.

Qualifications

• Active, unrestricted California license in an applicable mental health discipline.
• Willingness to obtain and maintain Washington State licensure.
• At least one year of experience in a Collaborative Care Model (CoCM).
• Experience with EHRs, patient registries, and clinical documentation systems.
• Strong communication, organization, and care coordination skills.
• Ability to work full-time Monday–Friday, 8:00 a.m.–5:00 p.m.

Responsibilities

• Assess patients and coordinate care with primary care and specialists.
• Screen and assess for mental health and substance use disorders using appropriate tools.
• Facilitate patient engagement, treatment participation, and timely follow-up.
• Provide brief, evidence-based behavioral interventions and monitor outcomes.
• Maintain patient registry and document progress in the EHR.
