Carbon Health Technologies Inc. is seeking a dedicated Mental Health Specialist to join our Collaborative Care Model team in a fully remote role.

You will coordinate mental and physical health care for an assigned caseload, monitor progress, and collaborate closely with primary care providers and a Psychiatric Consultant to adjust treatment plans as needed. Qualifications

• Active California license in an applicable mental health discipline.
• Willingness to obtain Washington State license.
• At least one year of experience working in a Collaborative Care Model (CoCM).
• Licensure as LMHC, LPC, LMFT or LCSW or equivalent.
• Experience with measurement-based care and outcome monitoring.
• Full-time remote schedule (Mon–Fri, 8:00–5:00).

Responsibilities

• Support an assigned caseload coordinating with medical providers and other mental health professionals.
• Screen and assess for common mental health and substance use disorders using clinical tools.
• Facilitate patient engagement, treatment participation, and timely follow-up.
• Educate patients on conditions, options, and care strategies.
• Coordinate care across primary care and behavioral health resources.
• Maintain patient registry and document clinical progress in EHR.
