Job summary

Oak Street Health is seeking a Behavioral Health Specialist to deliver care management and short-term psychotherapy for patients with behavioral health needs. The role supports Care Teams across clinics, coordinating care with psychiatric consultants and external resources.

The position may offer remote work days based on program metrics, with 40 hours per week and a competitive benefits package. Strong collaboration and documentation in EMR are essential.

Qualifications

• Master's degree in mental health counseling or related field from an accredited school.
• Highest clinical level license or local equivalent.
• Experience screening for mental health or substance use disorders.
• Experience in assessment and treatment planning for common mental health issues.
• Familiarity with brief, structured intervention techniques.
• US work authorization required.

Responsibilities

• Collaborate with primary care teams to identify patients for behavioral health referrals.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and coordinate with psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including warm handoffs for telehealth.
• Build relationships with primary care teams and communicate care plan changes.
• Document encounters and counseling sessions in the electronic medical record.
• Participate in regional and organizational efforts to advance the program.
• Possibly cover multiple clinics depending on panel sizes.
• Perform other duties as assigned.
