Job summary

Concert Health is seeking a Collaborative Care Clinician I to provide evidence-based interventions for pediatric and adult patients in a 100% remote setting. You will work under the Clinical Excellence Manager, delivering care within the Collaborative Care Model and coordinating with primary care providers to optimize outcomes.

The role requires an MSW/LCSW or equivalent with NY licensure, 1–5 years of licensed experience, and English/Spanish interpretation skills.

Qualifications

• Master’s Degree in Social Work or related field.
• Licensed to practice in New York (LMSW, LCSW or equivalent).
• Minimum 1-5 years experience as a licensed professional.
• A secure high-speed internet connection capable of supporting video conferencing and cloud-based systems.
• Proficient in medical interpretation and documentation in English and Spanish that meet industry standards.

Responsibilities

• Provides care to patients, based on needs of assigned Care Pod, customizing treatment plan to individual patient needs
• Supports and coordinates behavioral health care with the patient’s primary care provider and other service providers
• Screens patients for common behavioral health disorders using PHQ9, GAD7, CSSRS or other tools as required
• Monitors patients for changes in clinical symptoms and treatment side effects/complications
• Supports psychotropic medication management prescribed by PCPs focusing on adherence and effectiveness
• Documents patient progress and treatment recommendations in Salesforce and the EHR for sharing with care team
