Job summary

Integral Health is seeking a part-time Behavioral Care Manager to deliver direct individual therapy remotely to patients within a Collaborative Care Model. You will maintain a caseload, conduct screenings, and collaborate with primary care providers and psychiatric consultants.

Qualified clinicians hold a NY license (LMSW/LCSW/LMHC/LMFT or equivalent) with at least 2 years of licensed practice. This 1099 remote role offers flexible scheduling and growth opportunities.

Qualifications

• Master’s degree in a behavioral health field.
• Active New York license (LMSW/LCSW/LMHC/LMFT or equivalent).
• At least 2 years of full-time licensed mental health experience.
• Experience delivering direct behavioral health treatment or therapy.
• Commit to a minimum of 15 hours per week in a remote role.
• Strong clinical documentation and patient communication.
• Experience using EHR/EMR systems and clinical tools.
• Ability to manage a dynamic caseload remotely.

Responsibilities

• Provide individual behavioral health therapy and brief interventions.
• Maintain an assigned patient caseload and coordinate care.
• Conduct behavioral health screenings (PHQ-9, GAD-7, C-SSRS).
• Develop and update measurable, outcomes-based treatment plans.
• Apply CBT, Behavioral Activation, Motivational Interviewing, PST.
• Monitor progress and adjust plans with the care team.
• Coordinate with primary care providers and psychiatric consultants.
• Document progress, care coordination, and treatment plans in the EHR.
• Educate patients and families about conditions and resources.
• Participate in interdisciplinary care discussions and caseload reviews.
