Job summary

Oak Street Health in Columbus, GA is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy for patients with behavioral health needs. The role coordinates with Care Teams and a psychiatric consultant to assess, refer, and treat, with potential remote work days based on performance metrics.

The ideal candidate holds a Master's in a mental health field and licenses to practice independently, with experience in screening, assessment, and evidence-based

Qualifications

• Master's Degree in mental health counseling/healing arts from an accredited school.
• Highest clinical license to practice independently (LCSW/LPC/LMFT or local equivalent).
• Experience with screening for common mental health and/or substance use disorders.
• Experience with assessment and treatment planning for common mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., MI, Behavioral Activation, Problem Solving, CBT).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to the Oak Street Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultant or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant, including telehealth handoffs and care plan collaboration.
• Maintain relationships with primary care teams and communicate changes in care plans.
• Accurately document patient encounters and counseling sessions in the EMR.
• Participate in regional initiatives to advance the Behavioral Health Program.
