Job summary

Oak Street Health - Columbus GA seeks a Behavioral Health Specialist to provide care management and evidence-based short-term psychotherapy for patients with behavioral health needs. The role collaborates with primary care teams and a psychiatric consultant, with possible remote work days based on program metrics.

Responsibilities include developing care plans, referrals, counseling, and thorough documentation in the EMR.

Qualifications

• Master’s degree in mental health counseling, social work, marriage and family therapy, or a related field.
• Highest clinical level license to practice independently (LCSW/LPC/LMFT or equivalent).
• Experience screening for mental health and/or substance use disorders; experience with assessment and treatment planning.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify appropriate patients for referral to Oak Street Health’s Behavioral Health Program.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including telehealth handoffs and joint care planning.
• Build relationships with primary care teams and communicate changes in behavioral health care plans.
• Document patient encounters and counseling sessions in the EMR.
• Participate in regional and organizational efforts to advance the Behavioral Health Program.
