The Behavioral Health Specialist role at Oak Street Health Columbus, GA focuses on care management and short-term evidence-based psychotherapy for patients with behavioral health needs. You will work with Care Teams and a psychiatric consultant to coordinate care and referrals.

Ideal candidates hold an MS in a mental health field and licensure (LCSW/LPC/LMFT) to practice independently, with experience in screening, assessment, and brief interventions.

Qualificações

• Master's degree in mental health field from an accredited school.
• Current license to practice independently (LCSW/LPC/LMFT or local equivalent).
• Experience screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health disorders.
• US work authorization.

Responsabilidades

• Collaborate with primary care teams to refer patients to the behavioral health program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and arrange referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with the psychiatric consultant, including telehealth handoffs.
• Develop relationships with primary care teams and communicate changes in care plans.
• Document encounters in the EMR.
• Participate in regional and organizational efforts to advance the program.
• Possibly covering multiple clinics, depending on maturity and panel sizes.
• Other duties, as assigned.
