Qualifications

• Master's Degree in mental health field from an accredited school.
• Highest clinical license level to practice independently.
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health/substance use disorders.
• Familiarity with brief, structured interventions (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral).
• US work authorization.

Responsabilités

• Collaborate with primary care teams to identify patients for referral to the Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and facilitate referrals to psychiatric consultants and external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants and telehealth teams.
• Maintain timely documentation in the EMR.
• Participate in regional/organizational efforts to advance the program.
• Possibly cover multiple clinics depending on panel sizes.
• Other duties as assigned.
