Job summary

Oak Street Health seeks a Behavioral Health Specialist to provide care management and evidence-based short-term psychotherapy to patients with behavioral health needs. You will be a key resource for Care Teams across a region of Oak Street Health clinics, coordinating care with a psychiatric consultant and making referrals to internal or external resources.

The role offers potential remote work days based on program metrics.

Qualifications

• Master's degree in mental health counseling/healing arts from an accredited school.
• Highest clinical level of license (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for common mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral, etc.)

Responsibilities

• Collaborate with primary care teams to refer patients to Oak Street Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and provide/referrals to psychiatric consultant or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant, including telehealth handoffs and care plan collaboration.
• Develop relationships with primary care teams and communicate changes in care plans.
• Accurate documentation of patient encounters in the EMR.
• Participate in regional/organizational efforts to advance the program.
• Possibly cover multiple clinics depending on panel sizes.
• Other duties as assigned.
