Job summary

Oak Street Health seeks a Behavioral Health Specialist to provide care management and short-term psychotherapy across our primary care clinics. You will coordinate with care teams and a psychiatric consultant to assess needs, develop care plans, and refer to appropriate resources.

This role may include remote work days based on program metrics, and requires a Master's degree with licensure. Expect a 40-hour work week and comprehensive benefits.

Qualifications

• Master's degree in a mental health field from an accredited school.
• Highest clinical level license or local equivalent.
• Experience with screening and treatment planning for mental health/substance use disorders.
• Experience with brief structured interventions and psychotherapies.

Responsibilities

• Collaborate with primary care teams to identify patients for behavioral health services.
• Screen and assess for mental health and substance use concerns.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Document encounters accurately in the EMR and participate in program improvements.
