Job summary

MTC Care, a leading telemedicine provider for behavioral health in skilled nursing and assisted living facilities, seeks a full-time PCM Care Coordinator for a remote role. You will manage remote monitoring and principal care management for patients under medical supervision, with emphasis on data tracking and family engagement.

You will report monthly on clinical summaries and collaborate with the medical team to ensure timely interventions for approximately 15 patients daily.

Qualifications

• Must have at least a high school diploma.
• Strong organizational and follow-up abilities.
• Knowledge of medical terminology and clear written/verbal communication.
• Comfort with healthcare tech including EHR, Outlook, Excel and cloud tools.

Responsibilities

• Assist clinicians by gathering medical information for patients between visits using various EHRs.
• Monthly reporting of patient concerns such as falls, medications, behavioral issues, and hospitalizations.
• Proceed with PCM tasks for ~15 patients per day to ensure timely interventions.
• Escalate issues to the program manager or Chief Clinical Officer as needed.
• Support training of new employees through shadowing and guidance.
